Llanfyllin Group Medical Practice is based in three separate centres. Our doctors, nurses, Health Care Assistants and other health care professionals provide a wide range of clinics and services to over 10,000 patients from our main purpose built site in Llanfyllin and routine appointments from our 2 branch surgeries situated in Four Crosses and Llanrhaeadr.

• All our premises are easily accessible with disabled access and parking facilities
• We offer all core NHS funded General Medical Services to our registered patients as well as a range of enhanced services.
• We also offer a range of private services and clinics (such as insurance medicals, private sick notes and certain vaccinations) that are not funded by the NHS.

We welcome all new residents living in our practice area who wish to become patients of our practice (see map opposite).

If you wish to register with the practice you can download our New Patient Questionnaire and GMS1 form from the menu below. Please print out the forms onto A4 paper and once completed and signed forward to Llanfyllin surgery by post.

Please ensure you have sufficient medication from your previous surgery for 28 days as it can take a number of weeks for us to receive your medical records. You will need to provide us with an up to date summary of the medication that you are currently prescribed from your current surgery. If you have already moved away from your previous surgery before registering here and need medication, you will need to contact your previous surgery who can arrange to send a prescription to a local pharmacy where it will be dispensed.

• If so, and if you do not have enough medication for the next 4 weeks, please request a prescription from your existing surgery so that you do not run out.
• Contact your current GP surgery and request a prescription. This can be dispensed at your usual pharmacy, or you can ask that they post the prescription to your new address and you can take it to a local pharmacy yourself.
• Your existing surgery may be able to fax your prescription to a nearby pharmacy, we can provide you with details of pharmacies in the area if needed. It may be necessary to check this with the pharmacy first, as pharmacies not permitted to dispense certain medicines against a faxed prescription.
• Please ask your existing surgery for a ‘leaver’s report’ and bring this is in to us. You should do this even if you have sufficient medication, as it will allow us to see what medicines you currently take. This may be important if you need urgent care whilst we wait for the transfer of your medical records. If you do not have a current surgery in the UK, or cannot recall your last surgery, then please contact Health in Wales, NHS England, NHS Inform Scotland or NI Direct Northern Ireland who will be able to locate your last registered practice.
• If you routinely take non-prescription medicines such as paracetamol, antihistamines or an antacid and you can't find an open pharmacy, then supermarkets, newsagents and petrol stations usually stock a basic range of over-the-counter medicines.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
